I Came across this photoshop editing video tutorial a while ago an found his videos really simple and effective.

Scott Voelker - Photographer

In this video he goes through the process of how to make a cover up.
He uses a studio portrait in the example but the same method can be used in many other situations too.

I used a similar technique to get the photo of Scott for this post from the photo I had of him.
