In this video he goes through the process of how to make a cover up. He uses a studio portrait in the example but the same method can be used in many other situations too. Take a look at this video from fellow photographer Scott Voelker this is a very simple and effective technique that is easy to follow. I used a similar technique to get the photo of Scott for this post from the photo I had of him.
